You may also book your first appointment online. In-person and Virtual appointments are available for new and existing patients across Ontario.

You will be asked to complete an intake form before your first appointment and provide some information about your main concerns and health history. Your first visit will include a detailed consultation. The acquired information from your health history, physical examination, and reviewing laboratory test results (if applicable), is necessary to make the diagnosis and form an individualized treatment plan.
